SMQ Affidavit for MD/MS Admission - 2025-26

 Notice for Sikh Minority Candidates for admission to

PG Medical Courses 2025 at SGRDIMSAR

This is for information of all the PG aspirants under Sikh Minority Quota, that for eligibility under the said Quota, all documents and physical verification has been made compulsory to participate in the 1^st round of counselling.  Only those Sikh Minority candidates will be allowed to participate who are having Singh or Kaur with their names by birth or got incorporated the same along with supporting affidavit and 2 newspaper advertisement cuttings before 3^rd Nov. 2025  i.e. last day of portal for online application submission by Baba Farid University of Health Sciences, Faridkot.

Candidates are required to appear for physical verification  on  4^th Nov. 2025upto 3:00 PM with the below documents at Sri Guru Ram Das Institute of  Medical Sciences & Research, Vallah, Sri Amritsar. 

1. 1. Application form
   2. Fee receipt
   3. Sikh Minority Affidavit attested by 1^st Class Magistrate or Notary (Available on SGRD website)
   4. Neet Admit Card and Result card
   5. One passport size photo
   6. Matriculation Certificate / DOB                   
   7. Certificate of qualifying Examination (4) 
   8. Internship Completion                       
   9. MBBS Degree
   10. Permanent Registration
