All the qualified candidates are invited for the counselling of Allied & Healthcare Diploma programmes session 2026-27
The Counselling will be held on 07-07-2026 (Tuesday) in Biochemistry Conference Hall, SGRD Institute of Medical Sciences & Research, Sri Amritsar.
Reporting Time: 9:00 AM
The candidate must bring all the following documents at the time of counselling:

Printed copy of their application form. |

b) |
Admit Card and Result Card of Entrance Exam. |
|
c) |
Original matriculation examination certificate indicating the date of birth. |
|
d) |
Original matriculation examination mark sheet indicating “pass” in the subject of Punjabi of that standard; (For candidate from Punjab State only). |

e) |
Original Detail Marks Card of 10+1. |
|
f) |
Original marks sheet certificate of 10+2 or equivalent examination/ Printed copy of 10+2 result. |
|
g) |
Original Character Certificate from the school / college last attended. |
|
h) |
Original Migration Certificate from last attended Board. |
|
i) |
Four recent passport size photographs with name and father’s name written on the back side. (4 photos of Student, 1 photo of Father, 1 photo of Mother) |

j) |
Gap Year Affidavit (if applicable). |

k) |
Copy of Aadhar Card (Student, Mother and Father). |
|
l) |
Recent Domicile Certificate/ Bonafide Resident of Certificate. |

m) |
Affidavit of Sikh minority (applicable for Sikh Minority quota Candidates) |
|
n) |
Original Caste Certificate if Applied for SC/BC Category. |
|
o) |
Income Certificate if Applied for SC/BC Category. |
|
p) |
5 Post Dated Cheques. |
Note:
- Candidate must bring one clear bag with them.
- Presence of Parents/Guardian is compulsory along with candidate.
- Demand Draft of Rs. 23,000/- in the name of SGRD Charitable Hospital Trust University A/c payable at Amritsar.
