Counselling Postponed Notice for Allied and Health Care Professional Courses Diploma Programs
All the qualified candidates are invited for the counselling of Allied and Health Care Professional Courses Diploma Programs session 2025-26
The Counselling will be held on 03/07/2025 (Thursday) in Biochemistry Conference Hall, SGRD Institute of Medical Sciences & Research, Sri Amritsar.
Reporting Time: 9:00 AM
The candidate must bring all the following documents at the time of counselling:

Printed copy of their application form. |

Admit Card of Entrance Exam. |
|
c) |
Original matriculation examination certificate indicating the date of birth. |
|
d) |
Original matriculation examination mark sheet indicating “pass” in the subject of Punjabi of that standard; (For candidate from Punjab State only). |

e) |
Original Detail Marks Card of 10+1. |
|
f) |
Original marks sheet certificate of 10+2 or equivalent examination/ Printed copy of 10+2 result. |

g) |
Original Character Certificate from the school / college last attended. |
|
h) |
Original Migration Certificate from last attended Board. |
|
i) |
Four recent passport size photographs with name and father’s name written on the back side. (4 photos of Student, 1 photo of Father, 1 photo of Mother) |

j) |
Gap Year Affidavit (if applicable). |
|
k) |
Copy of Aadhar Card. |
|
l) |
Recent Domicile Certificate/ Bonafide Resident of Certificate. |
|
m) |
Affidavit of Sikh minority (applicable for Sikh Minority quota Candidates) |

n) |
Original Caste Certificate for SC/BC Candidates. |

5 Post Dated Cheques. |
Note:
- Candidate must bring one clear bag with them.
- Presence of Parents/Guardian is compulsory along with candidate.
- Demand Draft of.*Rs. 23,000/- in the name of SGRD Charitable Hospital Trust University A/c payable at Amritsar.
*The fee structure is provisional and is likely to get revised by the competent authority.
